The referring employee must be employed by City of Hope at the time of payout. |

Payout is as follows: 50% after the successful completion of 90 days of hire and 50% after the successful completion of one year of hire. |

Per Diem Referrals are not eligible for a referral bonus. Part-Time referrals will be pro-rated accordingly. |

Supervisors and above are eligible only for referrals to departments requiring different disciplines than their own. |

Employees involved in recruiting activities that are considered a normal and required function of their position are not eligible to participate. |
| • |
Self-referrals or sourced candidates (e.g. professional search firms) do not qualify. |

Temporary staff, consultants, students, third party contractors, Human Resources employees and those involved in recruiting activities are not eligible to participate in the program. |

The referred employee must identify the referring City of Hope employee on their employment application. All referrals must be confirmed by the recruiter. |

If duplicate referrals occur, the first referral received (based on the application date) will be eligible for the referral bonus. If more than one referring employee is listed on the application form, the referral bonus will be split evenly. |

Hiring decisions are at the sole discretion of Human Resources and the appropriate Hiring Manager, and will not be discussed with the referring employee. |
• |
If the referred employee leaves through termination or resignation, the referring employee
will not be eligible for the remainder of the bonus. |

Referral Bonus for Newly Hired RNs
If you’re a newly hired RN, you can earn a special bonus by referring a friend within your first 30 days of employment. To qualify for this bonus, RNs must have been made or accepted an offer and passed a background check. If your referral results in a successful hire, you will be eligible for the following awards:
$3,500 - Night Shift

ONCOLOGY EXPERIENCED NURSE BONUS –
Referral Bonus for Referral of Experienced Nurse with Oncology Expertise
Current employees of City of Hope who successfully refer an experienced Oncology RN are eligible for this plan, if the referral results in a successful hire. To qualify for this bonus, the RN candidate you refer must have at least 2 years of current oncology experience with skills, which are comparable to City of Hope Nursing staff. If this Nurse is a prior employee, the employee must have left the institution for a minimum of 2 years. This determination of skills will be made by the Hiring Manager/Director and Human Resources.

Current City of Hope employees who successfully refer an Information
Technology and/or Clinical Information Specialist professional are eligible
